0345(3) The licensor interviewed staff present and reviewed statements from staff and parents from the day of the incident. All staff stated that the child made it out of the building, unsupervised. For this reason, the licensor finds the violation likely valid. 0345(5)(b) The licensor interviewed staff and viewed the room. The staff member present in the classroom stated her classroom door alarm wasn’t working. The licensor observed the door alarm wasn’t working. For these reasons, the licensor finds the violation likely valid.
